Title:Electronic Palliative Care Coordination Summary in North Tyneside: Challenges and Benefits

Abstract: Palliative and End of Life patients often have thoughts about where their care should be and what it should look like, especially towards the end of their life. How do we let all services involved in their care know all about these preferences of care? North Tyneside has been involved in a pilot sharing this information in a timely electronic way with out of hours services. This talk will describe this service, how it works and the benefits it has brought to Palliative Care Patients in North Tyneside.
